Faculty Notification of Academic Dishonesty
This form is used, usually by course instructors, to report incidents of academic dishonesty. Once a course instructor and student meet to determine responsibility and penalty, if any, this form and supporting documentation are sent to the Associate Dean of the school in which the course is taught (Dr. Barbara J. Porter/College of Arts and Sciences; Dr. Karen Dutt-Doner/School of Education and Human Services; Dr. James S. Valone/Wehle School of Business). The Associate Deans are responsible for keeping records on academic dishonesty.

Associate Dean Notification of Academic Dishonesty
This form is used, usually by an Associate Dean, to communicate with a student following a report of Academic Dishonesty or an appeal of such a finding. It enables a student to know if any further action will be taken by the college on the matter.
